Body Surface Area: Why Height and Weight Together Matter
Body surface area (BSA) is exactly what it sounds like — an estimate of the total surface area of your skin, expressed in square meters. It's not a metric most people think about day to day, but it quietly plays an outsized role in medicine. Unlike BMI, which only tells you about relative weight, BSA combines your height and weight into a single number that approximates how much "surface" your body actually has. That distinction matters more than it might seem, because a lot of physiological processes — how fast a drug distributes through your system, how much heat your body radiates, how your heart's output should scale to your size — track much more closely with BSA than with weight alone.
The Formulas Behind the Calculator
This calculator uses two of the most established BSA equations in the medical literature. The first is the Du Bois formula, published in 1916 by Delafield Du Bois and Eugene Du Bois after they directly measured the surface area of a small number of subjects using paper molds and coated skin patches: BSA = 0.007184 × weight(kg)^0.425 × height(cm)^0.725. It's remarkable that a formula built from such a small, century-old dataset has held up so well against modern imaging-based validation studies. The second is the Mosteller formula, introduced in 1987 by Dr. R.D. Mosteller as a simpler alternative: BSA = √(height(cm) × weight(kg) ÷ 3600). Mosteller's version trades a small amount of precision for a formula that's far easier to compute by hand, which is exactly why it became the default choice in most modern clinical calculators and electronic health record systems.
How Clinicians Actually Use BSA
The single biggest application of BSA is chemotherapy dosing. Many cancer drugs have a narrow therapeutic window — too little and the treatment doesn't work, too much and toxicity becomes dangerous — so oncologists dose in milligrams per square meter of BSA rather than milligrams per kilogram of weight. This approach became standard because early researchers found that BSA correlated better with metabolic rate and drug clearance across a wide range of body sizes, from small children to large adults, than weight alone did. BSA also shows up in cardiology, where cardiac output is often normalized to BSA to produce the "cardiac index," and in burn medicine, where the percentage of BSA affected by burns directly determines fluid resuscitation volumes and treatment urgency.
Reading Your Own Result
For a general sense of where you land, most adult women average around 1.6 m² and most adult men average around 1.9 m², with the overall adult range typically falling between 1.6 and 2.0 m². Children have proportionally much smaller BSA — often under 1.0 m² — which is part of why pediatric dosing requires such careful calculation rather than simply scaling down adult doses. If you're curious why the calculator shows two numbers instead of one, it's because Du Bois and Mosteller rarely agree to the decimal point; for most adults the gap is only a few hundredths of a square meter, small enough to fall within normal measurement variation, but seeing both gives you a useful sanity check.
Important Caveats
A key limitation: this tool is built for curiosity and general education, not for clinical use. Real medication dosing requires a licensed physician or pharmacist working from validated medical software, verified height and weight measurements, and consideration of factors like renal function, obesity adjustments, and drug-specific dosing caps that a simple calculator like this one cannot account for. Neither formula was derived from a large, diverse modern population, and both can be less accurate at the extremes of height and weight — very short, very tall, or very high body-weight individuals may see more deviation from their true surface area. Treat your result as a ballpark figure, not a substitute for professional medical calculation.
